Note that this is far from finished. Many sections, especially in the second half of the book, are rather rough and some are even completely empty. The reasons for making it available are

1. At 400 pages I think there is already enough material to be useful, and
2. I need your help in improving it, and especially in finding errors and pointing out parts that need to be improved (email ).
